The Nurse as Family Doctor - NYTimes.com

The Family Health Clinic of Carroll County, in Delphi, Ind., and its smaller sibling about 40 minutes away in Monon provide full-service health care for about 10,000 people a year, most of them farmers or employees of the local pork production plant. About half the patients are Hispanic but there are also many German Baptist Brethren. Most of the patients are uninsured, and pay according to their income ? the vast majority paying the $20 minimum charge for an appointment. About 30 percent are on Medicaid. The clinics, which are part of Purdue University?s School of Nursing, offer family care, pediatrics, mental health and pregnancy care. Many patients come in for chronic problems: obesity, diabetes, hypertension, depression, alcoholism.

Doctors are trained to focus on a disease. Nurses are trained to think more holistically.

What these clinics don?t offer are doctors. They are two of around 250 health clinics across America run completely by nurse practitioners: nurses with a master?s degree that includes two or three years of advanced training in diagnosing and treating disease. By 2015, nurse practitioners will be required to have a doctorate of nursing practice, which means two or three more years of study. Nurse practitioners do everything primary care doctors do, including prescribing, although some states require that a physician provide review. Like doctors, of course, nurse practitioners refer patients to specialists or a hospital when needed.

America has a serious shortage of primary care physicians, and the deficit is growing. The population is aging ? and getting sicker, with chronic disease ever more prevalent. Obamacare will bring 32 million uninsured people into the health system ? and these newbies will need a lot of medical care. According to the American Association of Medical Colleges, the United States will be short some 45,000 primary care physicians by 2020.

The primary care physicians who do exist are badly distributed ? 90 percent of internal medicine physicians, for example, work in urban areas. Some doctors go to work in rural areas or the poor parts of major cities, treating people who have Medicaid or no insurance. But they are few.

In part it?s the money. Primary care doctors make less than specialists anywhere, but they take an even larger financial hit to treat the poor. Particularly in the countryside ? even with programs that offer partial loan forgiveness, it?s very hard to pay off medical school debt treating Medicaid patients, much less those with no insurance at all.

And the job of a primary care doctor today is largely managing chronic disease ? coordinating the patient?s care with specialists, convincing him to exercise or eat better. Poor patients can be a frustrating struggle. Compared with wealthier patients, they tend to have more serious diseases and fewer resources for getting better. They are less educated, take worse care of themselves and have lower levels of compliance with doctors? orders. Very few people start medical school hoping to do this kind of work. Those who do it may burn out quickly.

It might seem that offering the rural poor a clinic staffed only by nurses is to give them second-class primary care. It is not. The alternative for residents of Carroll County was not first-class primary care, but none. Before the clinic opened in 1996, the area had some family physicians, but very few accepted Medicaid or uninsured patients. When people got sick, they went to the emergency room. Or they waited it out ? and then often landed in the emergency room anyway, now much sicker.

Just as important, while nurses take a different approach to patient care than doctors, it has proven just as effective. It might be particularly useful for treating chronic diseases, where so much depends on the patients? behavioral choices.

Doctors are trained to focus on a disease ? what is it? How do we make it go away?

Nurses are trained to think more holistically. The medical profession is trying to get doctors to ask about their patients? lives, listen more, coach more and lecture less ? being ?patient-centered? is the term ? in order to better understand what ails them.

?I?ve been out of nursing school since 1972 and I still remember that when faculty members finished talking about the scientific parts of the disease they would talk about the psycho-social part,? said Donna Torrisi, the executive director of the Family Practice and Counseling Network, which has three clinics in Philadelphia. ?It?s not about the disease, it?s about the person who has the disease. While in the hospital you?ll often hear doctors refer to a patient as ?the cardiac down the hall.??

Younger doctors are no doubt better at this than their older peers. But the system conspires against them. The 15-minute appointment standard in fee-for-service medicine ? which pays doctors according to how many patients they see and treatments they provide ? makes it unlikely that doctors will spend time discussing a patient?s life in any detail. Physician reimbursement places a zero value on talking to the patient. But nurse practitioners are salaried, giving them the luxury of time. At the Family Health clinics, appointments last half an hour ? an hour for a new diabetic or pregnant patient.

Jennifer Coddington, a pediatric nurse practitioner who is a co-clinical director of Family Health Clinics, said that she spends a lot of time teaching patients and their families about their diseases and how to manage it. ?We want to know socially and economically what?s going on in their life ? their educational level, how are they making it financially,? she said. ?You can?t teach patients if you?re not at their educational level. And if a patient can?t afford something, what?s the point of trying to prescribe it? He?s going to be non-compliant.?

A physician might suggest that a patient lose weight and hand him a diet plan ? or refer him to a nutritionist. At the Family Health clinics, nutrition counselors ? graduate students at Purdue ? will sit down with patients to talk about the specific consequence of their diet, and suggest good foods and how to cook them, Coddington said. ?When you don?t have enough money to buy fruits and vegetables, so you go to the dollar menu at McDonald?s ? we help those people put planners together for the week.?

Data has shown that nurse practitioners provide good health care. A review of 118 published studies over 18 years comparing health outcomes and patient satisfaction at doctor-led and nurse practioner-led clinics found the two groups to be equivalent on most outcomes. The nurses did better at controlling blood glucose and lipid levels, and on many aspects of birthing. There were no measures on which the nurses did worse.

Nurse-led clinics can save money ? but not always in the obvious way. Many are cheaper than comparable physician-led clinics. Suzan Overholser, the business manager of the Family Health clinics, said that their cost per patient was $453 per year ? lower than the Indiana average for similarly federally qualified clinics (all the others physician-led) of $549. But nurse-led clinics aren?t always cheaper. Coddington examined published studies of clinic costs and found that in some cases, nurse-managed clinics had slightly higher per-patient costs than traditional clinics.

Although nurses are paid less than doctors (Medicare reimburses them at 85 percent of what it pays doctors,) nurse-led clinics are often very small, and so don?t have the variety of practitioners necessary to keep a clinic running at full capacity. They also serve the most difficult and expensive patients.

The biggest financial benefit, however, likely comes from offering patients an alternative to the emergency room. Coddington?s review cites studies showing large savings in paramedic, police, emergency room and hospital use. A traditional clinic in an underserved area would do that, too, of course ? it?s just that nurses tend to go where doctors won?t.

There are about 150,000 nurse practitioners in America today. The vast majority practice in traditional settings ? only about a thousand are in nurse-managed clinics. One reason these clinics are rare is that they may equal traditional clinics in health care, but not in business success.

Nurse-managed clinics have to overcome regulatory and financial obstacles that traditional clinics don?t face. Powerful physicians? groups such as the American Academy of Family Physicians oppose allowing nurses to practice independently. ?Granting independent practice to nurse practitioners would be creating two classes of care: one run by a physician-led team and one run by less-qualified health professionals,? says a paper from the A.A.F.P., citing the fact that doctors get more years of education and training. ?Americans should not be forced into this two-tier scenario. Everyone deserves to be under the care of a doctor.?

Only 16 states and Washington, D.C., allow nurses complete independence. In other states, some of the restrictions are bizarre ? in Indiana, for example, nurse practitioners may do everything doctors do, with two exceptions: they can?t prescribe physical therapy or do physicals for high school sports.

Jim Layman, the executive director of the Family Health clinics, said he thought that nurse practitioners cared for the majority of Medicaid patients in Indiana. But if you look through Medicaid records, you?ll find only doctors ? nurses are not allowed to be the primary caregiver of record. So the Family Health clinics, like others, employ a physician off-site from 4 to 6 hours a week who uses electronic health records to examine a sample of cases and consult when necessary. Medicaid is billed in his name.

It is not easy for nurse-run clinics to win status as a Federally Qualified Community Health clinic, which would allow them to get federal grants. This is largely because most come out of universities, and most universities don?t want to cede control to the community ? a requirement for this status. Purdue decided it would, and the Family Health clinics qualified in 2009. Before that, they received some money from the state, and raised the rest from local March of Dimes, United Way and Chamber of Commerce donations, plus fund-raising dinners and auctions. This was enough to support just one full-time provider at each clinic. Getting F.Q.C.H. status allowed them to hire more staff and move the Carroll County clinic into a modern new building ? and probably saved them from collapse. ?It would have been very difficult for us had we not gotten F.Q.C.H. status,? said Coddington. The Affordable Care Act ? Obamacare ? did authorize $50 million for five years for nurse-managed clinics. So far 10 clinics have gotten a total of $15 million.

In some ways, the nurse practitioner-managed clinic is a throwback to the small-town family practice, when your doctor asked about the schoolyard bully and your dad?s unemployment. Among the many changes needed in how America values and reimburses health care, it?s important to encourage and support these clinics. They may be old-fashioned, but that doesn?t mean they should be financed with bake sales.

China rare earths producer suspends output

BEIJING (AP) ? China's biggest rare earths producer has suspended production in an effort to shore up plunging prices of the materials used by makers of mobile phones and other high-tech products.

State-owned Baotou Steel Rare Earth (Group) Hi-tech Co. said in a statement released through the Shanghai Stock Exchange that it suspended production Tuesday to promote "healthy development" of rare earths prices. It gave no indication when production would resume and phone calls to the company on Thursday were not answered.

Beijing is tightening control over rare earths mining and exports to capture more of the profits that flow to Western makers of lightweight batteries and other products made of rare earths. China has about 30 percent of rare earths deposits but accounts for more than 90 percent of production.

Beijing alarmed global manufacturers by imposing export quotas in 2009. It also is trying to force Chinese rare earths miners and processors to consolidate into a handful of government-controlled groups.

Baotou Steel Rare Earth announced a similar one-month halt to production last October, also in an attempt to push up prices by reducing supplies.

The United States, the European Union and Japan have challenged China's quotas in the World Trade Organization as a violation of its free-trade commitments. Beijing has defended its controls as necessary to safeguard a scarce resource and minimize environmental damage from mining.

Rare earths are 17 minerals used in goods including hybrid cars, weapons, flat-screen TVs, mobile phones, mercury-vapor lights and camera lenses. The United States, Canada, Australia and other countries also have rare earths but most mining stopped in the 1990s as lower-cost Chinese ores came on the market.

The global economic slowdown has hurt demand, pushing down rare earths prices. Some manufacturers have been prompted by the Chinese controls to switch to alternative materials.

Beijing's trading partners complain its export controls push up rare earths prices abroad and give buyers in China an unfair advantage.

The price of one rare earth, lanthanum oxide, has fallen 65 percent on global markets since the start of this year to $15 per kilogram this week, according to Lynas Corp., an Australian miner. But that still is nearly double price of $9 per kilogram paid by buyers in China.

Other rare earths have shown similar price declines and a wide margin between prices in China and abroad.

Buffett says global economy slowing, wants more deals

(Reuters) - Warren Buffett sees clear signs that the global economy is slowing, although the U.S. economy is "inching ahead" as other regions decline, the Berkshire Hathaway chief executive officer said on Wednesday.

Yet despite the weakness, Buffett said he was "salivating" to make another major acquisition, adding that two deals of around $20 billion each had fallen through this year.

In an interview on CNBC, Buffett said the rate of decline was larger in Asia than in Europe, although the Asian economies are declining off a higher base.

At the same time, he said Berkshire's U.S. housing-related businesses were growing at double-digit percentage rates of late.

Buffett has maintained that the economy will not truly start to turn around until the housing market improves, and he said Wednesday that housing had clearly started to turn.

Berkshire is heavily exposed to housing construction, as it owns a brick maker, a carpet maker, a realtor and the largest builder of manufactured homes.

Yet even with growth from its existing portfolio companies, Berkshire is always on the hunt to expand by acquisition.

Buffett told CNBC: "I'm salivating" to spend some of Berkshire's $40 billion cash pile on a deal. At Berkshire's annual meeting in May, he said a $20 billion deal had recently fallen through, and on Wednesday he acknowledged a second one had come apart as well.

He did not give any indication as to what kinds of companies were involved.

In a wide-ranging interview from an event in Ohio, Buffett also said the best thing for the economy would be for U.S. Federal Reserve Chairman Ben Bernanke to stay on for a third term.

With the presidential race running neck-and-neck, there has been speculation of late that Republican candidate Mitt Romney, if elected, would seek a new Fed chairman when Bernanke's term expires in 2014. Buffett disagreed with the idea.

"I would vote for Bernanke again, and I'd get my kids out and everybody else to vote for him," he said.

WADI DINAR, Libya (AP) ? New fighting flared in a hilltop town in Libya on Tuesday between fighters defending Bani Walid, a stronghold of slain dictator Moammar Gadhafi's regime and pro-government militias trying to win control.

A military spokesman, Gen. Ali al-Shekhili, said there is still some resistance, but the pro-government forces have made significant advances toward the center of the town. Large numbers of residents have fled over the past few days, he said.

The violence in Bani Walid coincided with celebrations of the anniversary of the declaration of liberation of Libya from Gadhafi's dictatorship after an eight-month civil war.

A year later, Libya is still fractured by rival militias, tribes and armed backers of the old regime, and the government is struggling to impose its authority.

There was a brief lull in the fighting on Monday.

On Tuesday, snipers took over tall buildings in the center of Bani Walid, some 140 kilometers (90 miles) southeast of Tripoli, firing at pro-government militias on the town's outskirts.

The Libyan government's move to take control of Bani Walid came in the aftermath of the kidnapping, torturing and killing last month of an anti-Gadhafi fighter by Bani Walid forces.

The country's newly elected National General Council issued a decree to arrest the men behind the killing. Efforts for a peaceful resolution through negotiations with the town's elders failed.

An AP reporter at the outskirts of the town saw doctors and nurses fleeing. A Bangladeshi doctor, who refused to give his name for fear of retribution, said that the main hospital is not functioning, with no electricity and no medical supplies. Foreign workers were seen walking 20 kilometers (12 miles) to escape the gunfire.

As night fell, commander Muftah Abu-Lefa of Libya Shield, a militia under Defense Ministry control, told The Associated Press that his forces reached the hospital, and clashes had ceased for the time being.

Farid Moammar, another Libya Shield commander, accused fighters of Bani Walid of using residents as human shields and hostages to prevent the pro-government militias from advancing into the town.

BEIRUT (AP) ? Lebanese protesters erected flaming roadblocks and gunmen roamed the streets Saturday in a city on edge after the assassination of a top security official in a powerful car bomb the prime minister linked to the civil war in neighboring Syria.

The crisis raised a terrifying specter for Lebanese who fear their country could easily plunge back into cycles of violence and reprisal that have haunted it for decades.

Friday's blast in the heart of Beirut's Christian area killed eight people, including the country's intelligence chief, Brig. Gen. Wissam al-Hassan. It was the deadliest bombing in Beirut in four years, shattering the country's uneasy calm.

The government declared a national day of mourning for the victims Saturday, but protesters burned tires and set up roadblocks in anger.

Sharbal Abdo, who lives in the neighborhood where the bomb went off, brought his 6-year-old son, Chris, and 12-year-old daughter, Jane, to see the destruction Saturday.

"They were very afraid yesterday," he said. "They need to face this situation. It may be their future."

Lebanese Prime Minister Najib Mikati on Saturday linked the bombing to al-Hassan's high-profile investigation this summer that uncovered what authorities called a plot by Syria to provoke chaos in Lebanon with bombings and assassinations.

"I don't want to prejudge the investigation, but in fact we cannot separate yesterday's crime from the revelation of the explosions that could have happened," Mikati said at a news conference following an emergency Cabinet meeting.

Mikati, who opponents believe is too close to Syria and the Shiite militant group Hezbollah, offered to resign after the bombing, but was asked by President Michel Suleiman to stay.

Al-Hassan's probe led to the arrest of former Information Minister Michel Samaha, one of Assad's most loyal allies in Lebanon. Samaha, who is in custody, is accused of plotting a wave of attacks in Lebanon at Syria's behest. Indicted in absentia in the August sweep was Syrian Brig. Gen. Ali Mamlouk, one of Assad's highest aides.

Samaha's arrest was an embarrassing blow to Syria, which has long acted with impunity in Lebanon. Syria has powerful allies here, including the Shiite militant group Hezbollah, which now dominates the government.

For much of the past 30 years, Lebanese have lived under Syrian military and political domination.

Damascus' hold on Lebanon began to slip in 2005, when former Prime Minister Rafik Hariri was assassinated in truck bomb along Beirut's Mediterranean waterfront. Syria denied having any role. But broad public outrage in Lebanon forced Syria to withdraw its troops from the country.

The killings of anti-Syrian figures continued for years, however, and Assad has managed to maintain his influence in Lebanon through Hezbollah and other allies.

Now, as the Syrian civil war rages just across the border, Lebanon increasingly is getting sucked in.

Mikati said Saturday he had offered to resign after Friday's car bomb, but said Suleiman asked him not to plunge the country into more uncertainty.

The bombing raised fears that the crisis could unleash Lebanon's sectarian tensions, a dire scenario for a country that endured a devastating civil war of its own from 1975-1990.

U.N. Secretary-General Ban Ki-Moon spoke Saturday with Suleiman and stressed the importance of "dissociating the country from regional events" and in Lebanon's sovereignty, a U.N. spokesman said.

The Syrian unrest has already enflamed tensions here. Many of Lebanon's Sunni Muslims have backed Syria's mainly Sunni rebels, while Shiite Muslims have tended to back Assad. Al-Hassan was a Sunni whose stances were widely seen to oppose Syria and Hezbollah.

Hundreds of Sunni protesters marched in force through downtown Beirut Saturday, placing the blame squarely on Syria and Hezbollah for al-Hassan's killing.

"Hezbollah is a terrorist group!" they shouted.

Police were trying to identify the bombers and find out how they managed to target al-Hassan, an important security figure who traveled under great protection and who likely took more precautions following Samaha's arrest.

"We don't expect to reveal the crime within few hours," police commander Maj. Gen. Ashraf Rifi told Future TV. "The investigation is like a puzzle. You collect the pieces and put them together in a logical way."

Al-Hassan had many potential enemies.

Besides his investigation of Samaha, al-Hassan helped investigate the 2005 assassination of former Lebanese Prime Minister Rafik Hariri, a powerful Sunni figure. An international tribunal indicted four members of Hezbollah for Hariri's killing, although the group denies involvement.

His department also had a role in breaking up several Israeli spy rings inside Lebanon in recent years, Lebanese officials said.

Al-Hassan, 47, who was married with two children, is expected to be buried Sunday next to Hariri's tomb in downtown Beirut. His family arrived in Lebanon on Saturday on a private plane from Paris, where they live.

Lebanon's top Sunni cleric, Grand Mufti Mohammed Rashid Kabbani, condemned the assassination, calling it a "criminal explosion that targets Lebanon and its people." He also called for self-restraint, saying "the criminal will get his punishment sooner or later."

But many Lebanese were seething with anger.

In the eastern town of Marj, protesters tried to storm an office of the pro-Syrian Itihad group. Lebanese soldiers pushed them away, wounding five protesters, security officials said. Dozens of people who marched in protest in the border town of Moqueibleh came under fire from the Syrian side of the border, forcing them to disperse, the officials said.

The highway linking central Beirut with the city's international airport was closed, as well as the highway that links the capital with Syria, the officials said.

In the predominantly Sunni northern city of Tripoli, gunmen were roaming the streets on motorcycles and opening fire in the air.

The army issued a statement urging Lebanese to overcome the crisis and coordinate among themselves in order to give a chance to the "the criminal killers who tried through the crime to incite strife and split the country."

Search for alien life about to step up a gear

LONDON (Reuters) - It remains in the realm of science fiction for now but the discovery of a new planet just four light years away will reignite a race to find a twin of planet Earth that may host extraterrestrial life.

The step change comes as the most powerful telescopes ever built are about to enter into service and as ideas about where life could exist are being turned on their head. At the same time, scientific discussion about the possible existence of alien life is becoming more mainstream.

"I think scientists are very happy having a rational conversation about the likelihood of life out there," said Bob Nichol, an astronomer at Portsmouth University in Britain.

Nichol said this was partly driven by the discovery of new planets such as one identified this week in the Alpha Centauri star system, the closest yet outside our solar system.

Over 800 of these so-called exoplanets have been discovered since the early 1990s.

"An explosion in the number of planets makes it so much more likely," said Nichol, adding that the many formats in which life appears on Earth is indirect evidence, though not proof, that life is out there.

Researchers from the Geneva Observatory said the newest planet to be found was too close to its own sun to support life. But previous studies have suggested that when one planet is discovered orbiting a sun there are usually others in the same system.

Rival astronomers are now likely to start scouring Alpha Centauri for more planets, possibly in the habitable zone around its stars.

NEW EYES AND EARS

The technological eyes and ears that scientists have at their disposal are about to take a leap forward too, broadening and deepening their search.

Barring a surprise discovery of microbes on Mars, we will see alien life long before we are ever able to touch it.

"I think it is realistic to expect to be able to infer within a few decades whether a planet like Earth has oxygen/ozone in its atmosphere, and if it is covered with vegetation," Martin Rees, Britain's Astronomer Royal, told Reuters.

The next decade will see two record-breaking telescopes come on line; the Square Kilometre Array (SKA), a huge radio telescope sited in South Africa and Australia, and Europe's Extremely Large Telescope (E-ELT) that will sit on a mountain top in Chile's Atacama desert and be the largest optical telescope ever built.

Their main task will be to probe the origins and nature of galaxies, but they will also look for signs of life on planets that can now only be seen in the roughest detail.

"I think the capabilities of new telescopes means that the detection of an ETI (extraterrestrial intelligence) is more likely in the next few decades, than it was in the last," said Mike Garrett, general director of Astron, the Netherlands Institute for Radio Astronomy.

With a mirror almost 40 meters in diameter, The E-ELT will be able to reveal planets orbiting other stars and will produce images that are 16 times sharper than the Hubble Space Telescope.

When completed in 2024, the SKA radio scope will comprise 3,000 dishes, each 15 meters (50 feet) wide, together with many more antennae that together will be able to see 10 times further into the universe and detect signals that are 10 times older.

Among those signals could be radiation given off by military radar from the nearest million or so stars. "So," said Nichol, "if there are advanced civilizations on planets around those stars, we could see them".

Isobel Hook, an Oxford University astrophysicist who is working on the E-ELT, said the new telescope will boost the search for life elsewhere.

"The ELT should also allow us to study the atmospheres of extra-solar planets and look for ?bio-markers' such as water, carbon dioxide and oxygen molecules in their spectra," she said.

With the right equipment, Hook said the ELT may be able to use spectroscopy, the study of the particular wavelengths of light reflected by an object, to detect signs of vegetation on distant planets.

NEW THEORY

The search for alien life has long been framed by the dogma of a ?goldilocks zone' around faraway suns that is just the right temperature - neither too hot nor too cold - to allow liquid water, essential for life as we know it.

That theory is now being challenged, expanding the potential area in which life could exist.

Other sources of heat have been identified and scientists have come up with radically new ideas about the forms life could take after studying organisms that live in some of the most inhospitable places on Earth.

Earlier this year, Xavier Bon?ls of the Institute of Planetology and Astrophysics in Grenoble estimated there could be tens of billions of rocky planets in our galaxy alone with the right temperature to support life.

Meanwhile, researchers at Aberdeen University in Scotland are working on a computer model which suggests astronomers should sharply increase the number of planets they regard as capable of hosting life.

Although scientists would expect water on the surface of a planet outside a habitable zone to be frozen, researcher Sean McMahon has argued the heat generated from inside a planet could be enough for large underground reservoirs of liquid water.

The study of pockets of life on Earth in places not dissimilar to an area being probed on Mars by NASA's rover Curiosity has also altered conventional wisdom.

A team from Centro de Astrobiolog?a in Madrid led by Felipe Gomez studied organisms in the sun-baked Chott el Jerid salt pan in Tunisia, the acidic Rio Tinto in Southern Spain and the permafrost on Deception Island in Antarctica.

Another team from North Carolina State University recently published research on a single-celled organism that lives in a hot spring near Mount Vesuvius in Italy and is able to eat uranium and draw energy from it.

The research suggests that planets hostile to human life might nevertheless suit these so-called extremophiles, leading to what Duncan Forgan at the Royal Observatory in Edinburgh calls "a much more complicated and richer concept of the habitable zone."

But at roughly 100,000 light years across, the universe is a big place and even if there is intelligent life elsewhere sending out signals astronomers say the chances of not hearing them are still considerable.

WASHINGTON, D.C., October 17, 2012 ? A dramatic new survey released today by American Humane Association reveals the most compelling evidence to date of the vital and extraordinary bond between people and their pets, particularly between children and the animals they grew up with. Indeed, four in 10 individuals say that the loss of a childhood pet from injury, disease or old age continues to affect them as adults. And, an even greater number of adults (44.4%) note that they are still affected by a pet who was given away, ran away or had an uncertain disposition.

These were the major findings of ?People, Pets and the World We Share,? a survey conducted by American Humane Association?s Animal Welfare Research Institute to examine the lasting impact a pet has on a child and the remarkable bond and loyalty they share. Based on an email survey sent to its nationwide database of supporters and Facebook followers, respondents were asked to remember one animal from their childhood in answering the questions.

Dogs were by far the most popular childhood pet remembered, accounting for over 70 percent of responses. Cats were second at 23.8%, with the remaining including a myriad of animals including horses, rabbits and hamsters. 93.2% respondents were women, 44.8% were 30-49 years old, 43.2% over 50 with 12% under the age of 30.

Overcoming Grief Important Factor in Future Pet Ownership, Reducing Shelter Numbers

The survey backed the recent findings in a pet ownership study released earlier this year by American Humane Association and PetSmart Charities. In Phase One of the ?Keeping Pets (Dogs and Cats) in Homes Retention Study,? interviews with more than 1,000 previous pet owners revealed that lasting grief over the loss of a previous pet was a significant obstacle towards owning a new pet. Highlighting the emotional intricacies of the human-animal bond, one in five (20%) of previous dog owners and one in six (17%) of previous cat owners cited the loss of a beloved dog or cat as a reason they chose not to have another. Helping people to overcome their reluctance to own another pet in adulthood is vital to decreasing the three to four million adoptable animals who are being euthanized in our nation?s shelters.

For many children, the passing of an animal might be their first experience dealing with the death of a member of their family. Respondents were encouraged to provide comments about the animal they selected for the survey, and their emotional stories illustrate the lasting impact and irreplaceable bond they shared with their beloved pets.

Also illuminating is how parents of the respondents were remembered depending on the parents? involvement with the loss of a pet. Some comments speak to a parent?s role in nurturing the human-animal bond, and a child?s ongoing feelings of fondness toward their parents as the result, while others express bitterness toward a parent that gave away a pet without their child?s knowledge.

A sampling of these comments follows:

??I was only five when we had to euthanize [our pet] due to cancer. My parents did note tell me until they brought him home to bury in our backyard. I can still remember sitting there bawling. That was 36 years ago.?

??I was a lonely little girl, living with my father as my parents fought over my custody. This little dog was ALL I had? She was taken away from me, I never was told where to and I?ve never forgotten her? it was traumatic.?

? ?I remember the details very clearly of his sudden illness, going to the vet, him being euthanized, and bringing him home to be buried? I?ve always been thankful to my parents that the situation was handled with grace and dignity for our beloved Barney??

??To me, losing a pet is no different that losing a relative or friend. They become a part of your family, and although they can?t communicate in forms of human language, the love and the bond that is created cannot be denied. I?ve had to deal with the loss of two pets in my life and just like the death of a loved one, I grieved and still think about the time we had [together] often??

??Caring for a pet showed me the power of compassion and unconditional love??

??Fuzzy was my best friend, my confidant and kid all rolled into one. She helped me grow up and never feel alone? I love and miss her greatly to this day??

??My dog Stormy was there when I was brought home from the hospital and was by my side from that day on. He was my best friend, my protector, my playmate and my confidant. He died when I was 11 and my memories of him have never, and will never, die??

??my dog disappeared and I spent months searching for him. I still have his picture to this day and am still haunted [by his loss] He was devoted to me and me to him??

?I know it might sound strange but I find it harder to see an animal pass away than a human.?

??having a pet as a child and being given the responsibility of caring for a creature that was dependent on its humans left me with a lifelong love of all animals and sympathy for those that have been neglected or abused??

Oil prices fall as supplies rise

Oil prices hovered around $92 per barrel Wednesday after the government reported a bigger-than-expected increase in US crude supplies. Extra gasoline in storage projected lower demand, pushing oil prices down.?

Oil prices?gave up early gains and hovered around $92 per barrel on Wednesday after the government reported a bigger-than-expected increase in U.S. crude supplies.

Supplies rose by 2.9 million barrels last week. That was almost double what analysts had forecast, according to Platts, the energy information arm of McGraw-Hill Cos.

Gasoline supplies also rose. Analysts had predicted a decline.

The extra gasoline and?oil?in storage tends to push?prices?down, because it suggests that there's enough to go around, or that demand is low.

In midday trading on Wednesday,?oil?was down 7 cents to $92.02 per barrel on the New York Mercantile Exchange.

Oil?had been trading above $92 per barrel earlier in morning, as European stocks had a positive day, and hopes grew that Spain might be on the verge of requesting a bailout.

Gasoline?prices?at the pump fell almost 2 cents, to $3.756 per gallon, according to AAA.

The waffling?oil?prices?matched stock indexes that are also mixed in midday trading. Technology giants IBM and Intel both said customers are holding back. Economic weakness generally means less demand for?oil, pushing?prices?down.

Brent crude, which is used to?price?international varieties of?oil, fell 80 cents to $113.20 a barrel on the ICE Futures exchange in London.

In other energy futures trading on the Nymex:

? Heating?oil?fell 2 cent to $3.17 per gallon.

? Natural gas gained 5 cent to $3.48 per 1,000 cubic feet.

? Wholesale gasoline fell 6 cents to $2.79 per gallon.

Like so many of you, I have watched the first three debates of this year?s presidential election cycle with hopes that the American people would be presented with two clear choices for the direction of our nation and the leadership of our country. What we have seen, however, is only one choice, one direction forward and one leader that will return our nation to prosperity. With just twenty days to go, it is clear that Governor Mitt Romney is the leader that we need!

You see, despite President Obama?s campaign slogan of ?Forward,? his campaign has proven to be anything but forward-thinking. Last night, while Governor Romney clearly and concisely outlined his plans to move America forward over the next four years, President Obama failed yet-again to even discuss a second-term agenda. Preferring to resort to personal attacks, distortions and half-truths that even the most left-leaning of fact checkers couldn?t ignore, the President confirmed that the hope is gone and it is time for a change in leadership.
